Photograph and Video Release Form

This form is for those who would give permission to have video or pictures taken of them.

I do hereby give permission to the Fond du Lac Area Women's Fund, to photograph or videotape me/use a photograph or video supplied by me (or photo/video of any minors, whom I am legally responsible). I give this permission with the understanding that these photographs or video may be used in any of the Fond du Lac Area Women's Fund’s print pieces, advertisements, electronic communications, website, or social media spaces for informational and promotional purposes. The photographs or video may be viewed by a wide variety of audiences, which includes, but is not limited to the Fond du Lac Area Women's Fund staff, volunteers, donors, professional advisors, organizations, news media, the general public, etc. I have read this release form prior to signing it and I understand its content.
